Insurance Information

General Information:

We are pleased to let you know that Medicare and other insurance companies cover most surgeries for varicose veins, based on medical necessity. The only treatment which will not be covered by insurance is sclerotherapy which is used for cosmetic spider vein treatments.

Most managed health care plans, HMO’s, require referral/authorization. It is the patient’s responsibility to obtain such authorization and have the referral faxed to our office before their appointment. By arranging for the referral you will be able to avoid having to reschedule the appointment for a later date.

When scheduling for your initial consult and ultrasound we will obtain your insurance information. That information will be given to our insurance Coordinator who will verify your coverage and determine what benefits your particular plan has for you for the services we will be providing you. If your plan is subject to a deductible and coinsurance, we will contact you with what your responsibility will be for that initial appointment. When the doctor determines if you require any surgeries, we will obtain authorization and when complete, make sure that you get the cost sheet for whatever your responsibilities will be. Insurance issues can be very complex and we seek to always make those as understandable to you as possible and eliminate as much confusion as we can. We are aways happy to answer any questions you may have and consider it a privilege to walk through these issues with you and help navigate them for you!

Conservative therapy before surgery is often required by insurance companies and by Medicare. The main requirement is for the past use of between weeks to 3 months of medical grade compression hose, based on the medical policy of your individual plan. If you are purchasing hose it is very important that you get 20-30 mmHG compression, which meets most insurance requirements prior to approval for surgery, and is also what you will be required to wear after each surgery.
